Signs You Might Have Low Testosterone

Do any of these sound familiar?

If 3 or more of these sound familiar, your testosterone may be below optimal. A simple blood test at Paragon can tell you in 48 hours.

  • Persistent fatigue and reduced energy despite adequate sleep

  • Decreased muscle mass and increased body fat, particularly around the abdomen

  • Reduced libido and changes in sexual function

  • Cognitive changes, including brain fog, poor concentration, and memory concerns

  • Mood changes, low motivation, and irritability

  • Poor recovery from exercise and reduced physical performance

  • Sleep disturbances and difficulty recovering overnight

  • Metabolic changes, including insulin resistance and difficulty managing weight

A thorough laboratory evaluation is required before any treatment decisions are made. Symptoms alone are not sufficient for diagnosis.

Low Testosterone Is Often a Metabolic Issue,
Not Just a Hormonal One

Research has established clear connections between testosterone deficiency and a range of metabolic and cardiovascular conditions.

For many men, hormonal decline is not an isolated finding. It is part of a broader metabolic picture that, when addressed comprehensively, can lead to meaningful improvements in overall health.

Insulin Resistance and Type 2 Diabetes

Low testosterone is associated with reduced insulin sensitivity and an increased risk of type 2 diabetes. Studies show that men with low testosterone are significantly more likely to develop metabolic syndrome. Restoring testosterone to optimal levels can support improvements in insulin function and glucose metabolism.

Cardiovascular Health

Low testosterone has been linked to an increased risk of cardiovascular disease, including higher rates of hypertension, unfavorable lipid profiles, and arterial stiffness. Research suggests that men with testosterone deficiency may have a greater likelihood of adverse cardiac events over time. Restoring testosterone to physiologically normal levels can support improvements in vascular function and cardiovascular risk markers as part of a broader treatment strategy.

Body Composition and Obesity

Testosterone plays a direct role in regulating fat distribution and lean muscle mass. Men with low testosterone tend to accumulate visceral fat the metabolically active fat stored around the abdominal organs while simultaneously losing muscle tissue. This shift in body composition can accelerate metabolic decline. Optimizing testosterone levels, alongside appropriate lifestyle interventions, can help support a healthier ratio of muscle to fat.

Bone Density and Musculoskeletal Health

Testosterone is essential for maintaining bone mineral density in men. Chronic deficiency is associated with accelerated bone loss, increased fracture risk, and early onset osteoporosis conditions more commonly associated with aging women but increasingly recognized in men with hormonal decline. Addressing testosterone deficiency can be an important component of preserving long-term skeletal integrity and reducing musculoskeletal injury risk.

Testosterone replacement therapy is not a treatment for any of the conditions listed above. However, addressing underlying hormonal deficiency can be one component of a comprehensive approach to metabolic health. Dr. Garten evaluates each patient's full health picture before making any recommendations.

What We Test Before and During Treatment

Safe testosterone replacement requires a thorough understanding of your baseline health. We evaluate a comprehensive hormonal and metabolic panel, not just total testosterone, to ensure accurate diagnosis, appropriate candidacy, and responsible ongoing monitoring throughout your care.

  • Total Testosterone

  • Free Testosterone

  • Estradiol (E2) — critical for managing side effects

  • PSA (Prostate Specific Antigen)

  • Complete Blood Count (CBC) — monitoring hematocrit

  • Comprehensive Metabolic Panel

  • Lipid Panel

  • Thyroid (TSH, Free T3, Free T4)

  • DHEA-S

  • Hemoglobin A1C

  • Vitamin D

Our approach for Testosterone
Optimization at Paragon

There is no standard testosterone protocol that works for everyone. Your hormonal profile is shaped by your genetics, body composition, metabolic health, age, and lifestyle. What works for one person may be inadequate or excessive for another, even when starting testosterone levels appear similar. At Paragon, we take the time to understand your full picture before we build a plan around it.

01

KNOW YOUR BASELINE

Before any recommendations are made, we run a comprehensive hormonal and metabolic panel. For men this includes total and free testosterone, LH, estradiol, CBC, PSA, a full thyroid panel, vitamin D, and vitamin B12. Where needed, we also assess for insulin resistance. Nothing is assumed.

02

YOUR INITIAL CONSULTATION

We take a detailed health history, explore your symptoms, and understand your goals. We educate you thoroughly on andropause and TRT before reviewing your labs together and building your personalised treatment plan.

03

EARLY CALIBRATION

Four to eight weeks after starting therapy, we repeat the necessary labs to evaluate your therapeutic levels and how your body is responding. Adjustments are made based on your data and how you feel. Both matter equally.

04

LONG-TERM PARTNERSHIP

Once stable, we see you every four to six months. Repeat labs, a full clinical review, and any protocol adjustments needed. Our goal is to restore your testosterone to optimal, physiologic ranges and support your long-term health as your needs evolve.

When Testosterone Replacement May Not Be Appropriate

Testosterone replacement therapy is not right for everyone, and careful screening is an important part of our process. TRT is generally not recommended for men with the following:

  • Active or suspected prostate cancer

  • Hematocrit above 54 percent

  • Severe untreated heart failure

  • Active breast cancer

  • Uncontrolled or severe sleep apnea
